problème de wifi atheros 5007

Bonjour, j'ai essayé de suivre les instruction donnés sur le forum pour configurer ma carte wifi (atheros 5007) mais ca na marche pas. Je l'avais fait par le passer et ca avait fonctionné, mais la j'ai formaté mon ordi et je suis bloqué. Alors tout d'abord voici les informations demandées dans chaque discution :
